Please complete this application and the attached Waiver and mail it (with cheque payable to CVHBBA) to:

To ensure a professional image and continuity, table skirting will be provided.
If you require electricity, you must provide your own electrical cords.
We will do our best to accommodate specific table/booth location requests but cannot guarantee location.
Setup will start at 10:00 a.m. and must be completed by 12:30 p.m.
All exhibits must remain in place until 8:00 p.m.
The Evergreen Seniors concession on the lower level will be open from 10:00 a.m. – 3:00 p.m.
Please make your own arrangements for the dinner meal.
CANCELLATION POLICY:
If you cancel your table reservation after receiving an acceptance letter and before September 1st, 2011, you will receive a refund, minus a $50.00 cancellation fee. Cancellations after September 1st, 2011, will result in the forfeit of the entire fee. In the event of Show cancellation, you will receive a refund of the entire show fee minus $50.
